6. Barangay Clearance attesting the real name and residence of the accused
7. Certification attesting the length of residency
8. House Sketch - dated, certified, signed and sealed by the barangay captain
9. Certificate of Detention, if detained or arrested, or Affidavit of Voluntary Surrender, if not detained
10. Fingerprint (piano)
11. Specimen signature, at least 5 signatures
12. Affidavit of Undertaking
13. Valid government-issued identification card, original and xerox copy (back-to-back)
14. Original Copy of PSA Birth Certificate with attached receipt
15. If married, female, original copy of PSA Marriage Certificate with attached receipt

MCTC CLEARANCE - FOR EMPLOYMENT/BOARD EXAMINATION

1. Valid Police Clearance or NBI Clearance
2. Latest Cedula

MCTC CLEARANCE / NO PENDING CASE CERTIFICATION

1. Information/order/any document about the case
2. Authorization letter in case of processing through a representative
3. Valid identification card of the party and the representative

MCTC CLEARANCE/CERTIFICATION - FOR LAND-TITLING/TAX DECLARATION

1. Original latest tax declaration
2. Authorization letter in case of processing through a representative
3. Valid identification card of the landowner and the representative

NOTE:

1. No cash transaction. Payment is made through GCash or bank transfer.
2. Amount to be paid is ₱120.00 (₱90.00 certification fee + ₱30.00 convenience fee per transaction).
3. First-time job seekers are exempt from paying the fee pursuant to the First Time Jobseekers Assistance Act.
